On Friday, September 25, 2026, local time, Google released the release notes for version v5.09 of the “Google Health” app for Android and iOS.

The main highlight of the “Google Health” app v5.09 for Android and iOS is support for the new “Health Guardian” health monitoring feature, which has begun rolling out to users of the Google Pixel smartwatches “Google Pixel Watch 3,” “Google Pixel Watch 4,” and “Google Pixel Watch 5”. With the Android version v5.09, “Google Pixel Watch” series users can now set up the feature, and their first monthly health summary is scheduled to arrive in early October 2026.

Note that “Health Guardian” generates reports on three key features: “Blood Pressure Trends,” “Insulin Resistance Trends,” and “Sleep Breathing Quality.”

Additionally, this update includes reflecting workouts recorded in third-party apps and devices into “Cardio Load,” improvements to nutrition management features, expanded options for activity logging, and the restoration of lost step streaks. The “Google Health” app v5.09 for Android and iOS will be rolled out gradually over the course of a week starting today.

■ Support for “Health Guardian”
“Health Guardian” is now available for users of the “Google Pixel Watch 3,” “Google Pixel Watch 4,” and “Google Pixel Watch 5.” Once you set up “Health Guardian” on Android v5.09, you will receive your first monthly health summary in early October 2026, after which you can view the three metrics and trends in the “Health” tab.
  • “Blood Pressure Trends”: Designed to help you understand blood pressure patterns before they put a strain on your heart. It will appear in the “Heart” section of the “Health” tab if you wore your “Google Pixel Watch” for five or more consecutive days during September 2026.
  • “Insulin Resistance Trends”: Will appear in the “Metabolism” section of the “Health” tab if you wore your “Google Pixel Watch” both day and night for seven or more days during September 2026.
  • “Sleep Breathing Quality”: Shows how your nighttime breathing patterns affect your daytime energy. Daily and monthly trends will appear in the “Sleep” tab and the “Breathing” section of the “Health” tab.
■ Reflecting Workouts Recorded by Third-Party Apps and Devices into “Cardio Load”
  • Workouts recorded in third-party apps and devices and synced to “Google Health” now count toward “Cardio Load.” This applies only when heart rate data during the workout is recorded, and workouts recorded on devices such as Garmin or Apple Watch are also reflected.
■ Improvements to Nutrition Management Features
  • When creating custom foods, it is now easier to switch serving units and meal times.
  • Fixed a bug on the iOS version of the “Google Health” app regarding custom food creation and an issue where deleted custom foods continued to appear in the list.
■ Expanded Activity Logging Options and Restoration of Step Streaks
  • Expanded Activity Logging Options: More choices have been added for how to log completed activities in the “Fitness” tab. You can view all options by tapping the “Log Activity” button.
  • Restoration of Step Streaks: Fixed a bug where some past step streaks were lost. Data restoration is currently underway, and the missing step streaks will reappear soon.
“Google Health” App v5.09 Update Release Notes

Please note that the availability of each “Health Guardian” feature varies by country, region, and device, and utilizing some features requires a “Google Health Premium” subscription. Incidentally, the “Blood Pressure Trends,” “Insulin Resistance Trends,” and “Sleep Breathing Quality” reports provided by “Health Guardian” are intended for general wellness and informational purposes only, and are not intended to diagnose or screen for hypertension, diabetes, or sleep apnea.
