Google Health Guardian Rollout: Pixel Watch 3/4/5 Finally Set to Launch

On Thursday, September 24, 2026, local time, Google finally rolled out “Health Guardian”—a new suite of health and safety features for the “Google Health” app that was previously teased during the announcement of the next-generation Google Pixel smartwatch, the “Google Pixel Watch 5”—which uses AI to track subtle changes in the body in the background, such as blood pressure, insulin resistance, and breathing quality during sleep.

The newly launched “Health Guardian” features consist of three functions: “Blood Pressure Trends,” “Insulin Resistance Trends,” and “Sleep Breathing Quality.” They are available at no additional cost on the “Google Pixel Watch 3,” “Google Pixel Watch 4,” and “Google Pixel Watch 5.”

With these three features—”Blood Pressure Trends,” “Insulin Resistance Trends,” and “Sleep Breathing Quality”—AI analyzes data collected while wearing the compatible “Google Pixel Watch” series over a multi-week period, and delivers the results as a trend report in the “Google Health” app on the 1st of every month. The first trend reports are expected to arrive in early October 2026.

However, to receive trend reports, users need at least 7 days of continuous wear data per month for “Insulin Resistance Trends,” and at least 5 days of continuous wear data per week for “Blood Pressure Trends.”

The AI-driven background tracking works differently for each feature. “Blood Pressure Trends” estimates blood pressure tendencies based on patterns of pulse and body movement measured throughout the month, eliminating the need for a cuff or manual calibration. It is powered by Google’s foundational AI model, “WavesFM,” which was trained on billions of minutes of raw heart rate and body movement data.

“Insulin Resistance Trends” uses a large-scale sensor foundational AI model pre-trained on over 1 trillion minutes of sensor data from 5 million consenting users to track changes in insulin resistance over multiple weeks without requiring a blood draw. Its accuracy has been validated against standard blood tests and published in the academic journal “Nature.”

“Sleep Breathing Quality” displays the percentage of sleep time spent breathing optimally every morning. It also provides a monthly report summarizing long-term trends.

In addition to these three features, the “Health Guardian” suite includes a safety feature called “Respiratory Emergency Detection.” This feature automatically places an emergency call if a user’s blood oxygen levels drop critically and they become unresponsive.

However, this is limited to the “Google Pixel Watch 4” and “Google Pixel Watch 5,” excluding the “Google Pixel Watch 3.” Furthermore, availability is restricted to certain European countries that have obtained the CE mark indicating conformity with EU safety standards.

Later in 2026, “Blood Pressure Trends” and “Insulin Resistance Trends” are scheduled to be rolled out as part of “Health Guardian” for the Fitbit screenless wearable tracker, “Fitbit Air.” However, “Health Guardian” on the “Fitbit Air” will require a paid subscription to “Google Health Premium.”
