On Thursday, October 1, 2026, Google released details regarding the “Metabolic Stress Level” feature, part of the “Health Guardian” health monitoring feature that finally began rolling out in late September 2026 for users of the “Google Pixel Watch 3,” “Google Pixel Watch 4,” and “Google Pixel Watch 5.”

“Health Guardian” is a new set of health and safety features that uses AI to track changes in the body in the background that are usually difficult to notice, such as blood pressure, insulin resistance, and nighttime breathing quality. It deploys three features: “Vascular Stress Level,” “Metabolic Stress Level,” and “Sleep Breathing Quality.” AI analyzes the data collected while wearing the “Google Pixel Watch 3,” “Google Pixel Watch 4,” or “Google Pixel Watch 5” over periods of several weeks, and delivers the results as a report in the “Google Health” app on the 1st of every month.

Specifically, the “Metabolic Stress Level” feature within “Health Guardian” monitors whether there is a trend toward increasing insulin resistance without requiring a blood draw. Insulin resistance itself is an indicator where cells in muscles and fat become less responsive to the action of insulin—even though enough insulin is being secreted by the pancreas—making it difficult for the body to properly absorb glucose.

Insulin resistance often progresses with almost no noticeable symptoms, and it is said that over 40% of adults in the United States live with undetected insulin resistance for years. While insulin resistance is a major risk factor for prediabetes and type 2 diabetes, blood tests have typically only been performed after a physician raises concerns.

In other words, it was often discovered only after abnormal values were confirmed during a routine health checkup. The “Metabolic Stress Level” feature in “Health Guardian” is designed to provide advance notice of such hard-to-notice insulin resistance before it reaches that stage.

The “Metabolic Stress Level” feature combines profile information such as height and weight with heart rate rhythms, sleep quality, daily activity levels, breathing rate, skin temperature, and blood oxygen saturation (SpO2) measured by the “Google Pixel Watch” series throughout the month, allowing Google’s proprietary AI model to estimate trends in insulin resistance. This AI model has been validated against the results of traditional clinical tests, and is capable of generating reliable estimates for general wellness use.

If the “Metabolic Stress Level” feature detects a sustained state of high insulin resistance, users are notified in the report delivered on the 1st of every month. Furthermore, “Google Health Coach,” the AI coach in the “Google Health” app, provides personalized and concrete advice—such as suggestions for foods to incorporate into your diet, tips for establishing a regular sleep schedule, and workout recommendations that naturally fit your physical condition and schedule—allowing you to immediately take action to restore your metabolic state to an optimal condition.

As mentioned above, while “Health Guardian” is currently available to users of the “Google Pixel Watch 3,” “Google Pixel Watch 4,” and “Google Pixel Watch 5,” “Vascular Stress Level” and “Metabolic Stress Level” are also scheduled to be rolled out as part of “Health Guardian” for the Fitbit screenless wearable tracker “Fitbit Air” within 2026.

However, using “Health Guardian” on the “Fitbit Air” requires a subscription to the paid “Google Health Premium” service.
